Managed Healthcare System - Schedule Of Benefits

The MHS Scheme has been withdrawn with effect 1 January 2008 for new applications
(applicable to both Individual and Group policies).
Existing MHS policyholders will continue to enjoy their coverage as long as
they renew their policies annually.

Benefits & Copayment By Member
A) Primary: Consultation, medication, basic diagnostic tests, x-rays and procedures
- Appointed GPs $5 co-payment per visit
- Non-appointed GPs Reimburse up to $10 per visit, subject to maximum 3 visits per year
- Specialised Investigations Co-payment of 10 %
B) Specialist Care: Consultation, medication, basic diagnostic tests, x-rays and procedures, up to $500 per year
- SOC at restructured hospitals
(Visit must be referred by appointed GP)		 $15 co-payment per visit *

- Private specialists
(Visit must be referred by appointed GP)

 $15 co-payment per visit,
reimburse up to $100 per visit *
- Specialised Investigations Co-payment of 10 %
* excluding those procedures found under section (D)
C) Emergency:
  
- Restructured hospitals $10 co-payment per visit
- Private hospitals & clinics $10 co-payment per visit, reimburse up to $70 per visit
D) Hospital Care Co-payment 10% of total eligible bill
